To apply for assisted housing, you may go to our online application.   

At the time of application, you will need the following information:

• Proof of Social Security number for all household members
• Photo identification of all household members
• Income verification - letter or print out from employer, child support, SS, SSI, and VA contribution letter
• Birth certificate

FSHA staff will review your application and verify that all areas of the application are complete. If an applicant needs help in order to complete the application process, such as needing the application read aloud or interpreted, a FSHA representative will be able to assist. After you have been determined eligible, you will be notified and your name will be placed on the waiting list. Please feel free call the FSHA to verify your waiting list position at any time.

When an opening becomes available we want to be able to notify you as quickly as possible. Should your contact information change while you are on our waiting list, we ask that you immediately update your appliation profile to insure we can make contact with you.
